Care Matters is dedicated to helping individuals and their families to cope with traumatic injuries,
illnesses, and chronic conditions. We provide solution-focused medical case management and social
work support services from a strength perspective to promote recovery and rehabilitation. Family
members and friends will find their role as a caregiver to be eased as Care Matters carries the load.
Today's work climate can be complex, demanding, and stressful. Employees and their families often need
employee assistance programs (EAPs) to resolve stress and other concerns that negatively impact their lives
and ultimately their job performance. As an employee advocate, Care
Matters provides access to community resources and helps make decisions which will improve the employees
current level of functioning.
Care Matters' services are value-added for guardians/legal representatives of a person injured in an
auto accident or job incident. We help increase the effectiveness of services by helping injured
persons and their families to navigate the medical system and access community resources. We collaborate
with healthcare professionals and monitor the patient's recovery or rehabilitation progress.
Care Matters supplements the treatments provided by doctors, nurses, therapists, and other
healthcare professionals. Providers can help further their patients' well-being by referring the
patient to Care Matters for professional case management and counseling. We implement a care plan based on a
biopsychosocial assessment which allows us to recognize and resolve barriers that inhibit positive
treatment outcomes.
