What Is Medical Case Management?

Medical Case Management refers to the planning and coordination of healthcare services to achieve recovery, rehabilitation and overall social well-being.

Medical Case Management is a collaborative process that facilitates recommended treatment plans to ensure the appropriate medical care is provided to disabled, ill or injured individuals.

Medical Case Management may include, but is not limited to, evaluating the status of a person's medical condition, developing and implementing a plan of care, coordinating medical resources, communicating healthcare needs, monitoring an individual’s progress, and facilitating cost-effective care.
